LIST OF DOCUMENTS:
1. Online Nonimmigrant Visa Application (DS-160) at http://armenia.usembassy.gov/step_by_step_iran.html
2. If your child is immigrating with you, DS-160 for the child must be filled out too.
3. A valid travel passport with 2 copies of data containing pages.
4. An original birth certificate with its photocopy. If your original documents are not in Armenian or Russian, you must present a notarized English translation of the document.
5. Evidence of termination of prior marriage(s), such as divorce certificate (if applicable), death certificate of spouse (if applicable): original, photocopy and translation into English if your original documents are not in Armenian or Russian.
6. Applicants are required to submit police certificates in all names as well as all dates of birth that they have used. The police certificate must be submitted from each place in which the applicant lives or has lived for more than six months since reaching the age of 16. This includes localities where applicants have lived during university studies. If the applicant was on military service, he/she should bring the certificate from the local draft board. If an applicant has lived abroad for more than one year, a police certificate must also be submitted from the country in which he/she lived. Police certificates for Armenia are issued by the Armenian Foreign Ministry’s consular department. No police certificate is needed from Iran.

8. Accompanying child requires a valid passport, a birth certificate and a medical examination. If a child is 16 years of age or over, police certificates are required. Translations of the indicated documents are required if your original documents are not in Armenian or Russian.
9. Documents confirming relationship: photos of Petitioner and Beneficiary together, letters to each other, phone bills, airplane tickets, emails, etc. (K3 applicants should present marriage certificate).
10. All the applicants must pass a medical examination. The medical exam must be conducted by one of the Embassy's panel physicians (see the attached list). It takes approximately 2 days to process the results of the medical exam. For more information regarding the medical exam please contact one of the medical centers directly.
11. Two color photos full face 5x5 cm for visa (another three pictures will be required for medical exam).
12. Financial documents from your US citizen fiancé(e): Original I-134 Affidavit of Support, a copy of tax returns (Form 1040) and W-2s for the last year. If tax returns are not available, provide transcripts from the IRS.
Financial documentation from the petitioner may also include but is not limited to employment letter for the petitioner with salary information and/or bank statement.

A medical examination should be conducted before the immigrant visa interview by one of the panel physicians. The following is information about the panel physicians in Yerevan authorized to conduct these examinations. When you go to the clinic, please be sure to bring the following: 3 color photographs 3X4 cm in size, your travel passport, and an extract from your medical history listing the vaccinations you have received, signed and stamped by your medical doctor.
The results of the examination should be presented to the Consular Section in Yerevan. Please call the panel physician directly to schedule an appointment; all questions regarding medical appointments and the examination should be directed to the panel physicians. Please note that the cost of vaccinations is not included in the cost of the medical examination.
